90-minute Initial Interview Visit

Prior to the Initial Interview Visit, you will complete and submit a Client Disclaimer, as well as the following key tools:

Initial Interview Questionnaire

Collects in-depth personal nutrition, and lifestyle information.

Food & Mood Journal

Captures all the food, beverages, supplements, and medications consumed over a multiple-day period.

NAQ Nutritional Assessment Questionnaire

Composed of questions regarding your personal nutrition and lifestyle.

During your Initial Consultation, we will conduct an in-depth review of your submitted forms, allowing us to explore your health history, current concerns, and wellness goals in greater detail. This session is designed to clarify any key insights from your responses, address pertinent questions, and establish a personalized roadmap for your nutritional healing journey.

 

Together, we’ll begin identifying potential root causes and opportunities for dietary, lifestyle, and supplemental support tailored to your unique bio-individuality. This collaborative process ensures a thorough, client-centered approach to achieving optimal health.

60-minute Personalized Recommendations Visit

Following your Initial Interview, I will conduct a thorough analysis of your health history, dietary habits, and wellness goals gathered from both your Intake Forms and our discussion.

Your custom nutrition therapy protocol

Using a root-cause approach, I will develop a customized nutritional therapy protocol tailored to your unique bio-individual needs. During this visit, we will review your personalized action plan, prioritizing the health goals you wish to address through targeted nutritional and lifestyle strategies. You’ll gain a deeper understanding of the underlying imbalances contributing to your symptoms and receive clear, actionable steps to restore optimal function.

 

My recommendations may include dietary adjustments, targeted supplementation, stress management techniques, and lifestyle modifications — all designed to support your body’s innate healing capacity. This session is your roadmap to sustainable wellness, empowering you with the knowledge and tools to achieve lasting vitality.

 

You will receive a comprehensive write-up of my recommendations, including dietary details, lifestyle modifications, and supplemental support (if applicable). Between visits, you’ll have the opportunity to implement these changes while observing and documenting any shifts in your symptoms or overall well-being. This process ensures we can fine-tune your protocol at your follow-up visit for optimal results.

To track changes in your symptoms and overall well-being.

A Food & Mood Journal

To identify connections between diet, lifestyle, and how you feel.

The Nutritional Assessment Questionnaire (NAQ)

To evaluate shifts in your nutritional status.

This updated data allows me to analyze the effectiveness of our previous protocol, determine what’s working well, and identify areas where adjustments may enhance your outcomes. During our session, we will discuss:
